Sick Leave Letter

A quick text to your manager works for one day out - but when an illness will keep you off work for days or longer, HR wants formal written notice with dates. It protects you too: a dated letter starts the clock on any paid sick leave you've accrued, creates the paper trail FMLA and disability claims lean on, and heads off any no-call, no-show misunderstanding while you're recovering. This letter states your first day out and expected return, confirms you can provide medical documentation on request, and shows you've thought about coverage - which is all a reasonable employer needs. Fill in the dates and your manager's name below, and get back to resting.

[Manager or HR contact name]

Dear [Manager or HR contact name],

I am writing to formally notify you that I need to take sick leave for a medical condition, beginning [First day of leave]. Based on current guidance from my doctor, I expect to return to work on [Expected return date]. If my recovery requires any change to that date, I will inform you as soon as possible.

I can provide medical documentation supporting this leave upon request, in line with company policy. Please let me know whether you or Human Resources need a doctor's note or any completed forms, and where to send them.

To keep things running while I am out, I have prepared notes on the current status of my work and pending deadlines, and I am glad to brief a colleague before or during my leave as my condition allows. For anything urgent, I will check email when I am able, though my responses may be delayed.

Thank you for your understanding. I will confirm my return date with you before [Expected return date].

Sincerely,

[Your full name]

Three tips before you send

  1. Email this the same day you know you'll be out - most company policies and state sick leave laws require notice as soon as practicable, and the timestamp proves you gave it.
  2. Ask your doctor for a work excuse note at the appointment, not after. Getting one retroactively can take weeks, and HR documentation deadlines are often only 15 days.
  3. If this could stretch past a few days, ask HR about FMLA and short-term disability now - the paperwork has strict deadlines, and protection isn't automatic if you never raise it.

Questions people ask

Can my employer require a doctor's note?

Generally yes, especially for absences longer than a few days - check your handbook for the trigger. Some states and cities with paid sick leave laws limit when a note can be demanded (commonly only after three consecutive days) and bar employers from requiring the diagnosis itself. You usually only need to confirm you were seen and are cleared for the dates.

Do I get paid while I'm on sick leave?

It depends on where you work. More than a dozen states and many cities require employers to provide accrued paid sick leave; elsewhere it's up to company policy. Longer illnesses may be covered by short-term disability insurance if you have it. Federal law doesn't require paid sick leave, so check your handbook, your state law, and your benefits summary.

When does FMLA apply to my sick leave?

FMLA covers a serious health condition - typically one involving inpatient care or ongoing treatment - not a routine cold. To be eligible you need 12 months with the employer, 1,250 hours worked in the past year, and a worksite with 50+ employees within 75 miles. If you qualify, FMLA provides up to 12 weeks of unpaid, job-protected leave. Tell HR you may need it; that triggers their paperwork duties.
